Florida Form SR22 is the name of the document that provides verification of required insurance for drivers with a suspended license. Depending on the circumstances, court suspensions have different terms of insurance, such as limits of coverage, compliance period, and terms of payment. An assigned case number, which appears in the Motor Vehicle Report (MVR), links insurance policies to drivers requiring SR22.
